Pre-Course Qualifications
For your application to be accepted:
- You should be a qualified Speech and Language Therapist, Nurse or other Healthcare Professional.
- You should have some basic knowledge, prior experience, or be working with appropriate client groups (e.g. adults, paediatric or adults with learning disabilities) with dysphagia.
- You should identify a suitably qualified supervisor (or supervisors) for the Supervisory Period of the course. We are unable to provide supervision or assist applicants in securing a supervisor. Please note that your supervisor must invigilate, or appoint an invigilator, on the Examination Day. It is possible to apply without first identifying a supervisor but you won’t be able to complete the Supervisory Period of the course without a supervisor.
- You MUST provide the information we require to issue our invoice (see invoice instructions below).
If you are unable to meet these criteria, please contact us to discuss your situation before applying.
If you are a manager/employer and you don’t yet know which employee will be attending the course, perhaps because you are recruiting, you can reserve a place by providing full and complete invoice instructions, including a purchase order number if required by your organization (see invoice instructions below). The completed application form can follow once the attendee has been identified.
Invoice Instructions
If you are self-funding, we will issue our invoice to the name and address given on the application form.
If your employer or other organization is funding your place, please note that we get lots of requests to reserve places while invoice details, purchase order numbers, etc are obtained from the paying organization. But the fairest way for us to operate for all concerned is on a strict “first come first served basis”. So we will ONLY accept application forms with full and complete invoice instructions, including a purchase order number if required by the paying organization.
So, if your employer or other organization is paying your fees, you must:
EITHER send the purchase order or other proof that funding has been agreed (which must include full invoice instructions including a purchase order number if required by your organization) with your application form
OR provide the correct information on the application form that the payer requires us to include on our invoice to ensure it will be paid within 30 days as follows:
- The name of the organization paying your fees EXACTLY as it should appear on our invoice.
- The name and postal address to send our invoice to.
- An email address that we can email our invoice to (if possible).
- Any PURCHASE ORDER NUMBER, or any OTHER REFERENCE NUMBER, that the paying organization requires us to quote on our invoice.
We cannot reserve a place on the course without the correct information to enable us to issue an invoice that will be paid within 30 days. Providing incorrect or incomplete information can lead to our invoice being unpaid and the loss of your place on the course.
Payment of Fees
If you are self-funding, immediate payment of our invoice is required.
If your employer or other organization is funding your place, payment is due within 30 days of issuing our invoice.
Fees can be paid by bank transfer, cheque, bank draft, credit card, debit card or through a Paypal account. There is a 4% surcharge for payment by card or Paypal.
Application Procedure
Click on the link below to download the application form as a Word document that you can fill in on your computer, save on your computer, then send to us attached to an email. Our email address is given on the application form.
After clicking the link above, look for dysphagia_course_application.doc in your downloads folder.
On receipt of your completed application form, the course tutors will assess your application to ensure that this dysphagia course is appropriate for you.
We then issue our invoice as per the instructions given on your application form/purchase order. If you do not provide full and complete invoice instructions (including a purchase order number if required by the paying organization), we can not reserve you a place on the course.
We then inform you by email that a place on the course has been provisionally reserved for you, subject to receipt of immediate payment (if you are self-funding) or payment within 30 days (if your employer or other organization is funding your place). This is usually done within one working day. If you do not receive an email from us, we haven’t received your application form.
Once payment is received, we will inform you by email that your place on the course is confirmed.
If your application is accepted, but the course you applied for is full, you will be added to the waiting list and given priority over subsequent applicants for the next available dysphagia course.
