We need to know which client group you want  to be examined on and make sure that we have up to date contact details for you and your supervisor.

So please fill in and submit the form below by the 10th January.

Please use the form below as it ensures we get all the information we need in the correct format. PLEASE DO NOT EMAIL THE INFORMATION.

If your supervisor intends to appoint someone else to invigilate your exam, AND would like us to email the exam paper to the invigilator DIRECTLY (not to the supervisor), then your supervisor should complete the Appoint An Invigilator form. They will need your student number (see label on front of course notebook). You still need to complete the form below as well.

Please note that we don’t acknowledge receipt of the completed form. If we haven’t received it by the date shown above, we will contact you. So if you don’t hear from us, it means we have received your information.
